Description
MAIN FUNCTION:
-Responsible for assessing clients to determine appropriate level of care.
-Responsible for completion of assessment and admission documentation.
-Responsible for making referrals as necessary.
-User role(s) information systems access as assigned.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
-Perform all aspects of client intake process including but not limited to admission- assessment, communicable disease risk assessment, CIWA Scale, COWS Scale Mental Status Exam, Intake & Referral and Individual Session.
-Completion of all OASAS required admission documentation including LOCADTR and consent forms.
-Determine appropriate level of care.
-Coordinate all inpatient referrals with Intake and Referral and the Utilization Management Department.
-Communicate with other Departments (i.e. Medical, Fiscal, Intake & Referral, Clinical, Administration, etc.) to relay important clinical information.
-Provide individual, group and family counseling as needed.
-Enter Progress Notes and other Clinical documentation into EMR.
-Participate in staff meetings.
-Facilitate the completion of all necessary consent for release of information forms
-Act as liaison with Community Agencies to facilitate referrals
-Participate in weekly Case Conference meeting for treatment review of clients.
-Attend weekly clinical supervision.
-Ensure that Seafield’s policies and procedures and federal regulations regarding confidentiality of the client and their records are adhered to.
-Perform other duties as assigned.

EDUCATION AND QUALIFICATIONS:
CASAC-T, CASAC, MSW, LMSW, LCSW, MHC (w/Limited Permit), LMHC. Clinical Experience in the field of Chemical Dependency. Working knowledge of Electronic Medical Records and Managed Care.
