MAIN FUNCTION:
- Responsible for assessing patients to determine appropriate level of care.
- Responsible for completion of assessment and admission documentation.
- Responsible for making referrals as necessary.
- Understanding of Addiction, Medication for Addiction Treatment (MAT), and the stigmatism associated with terminology and treatment of addiction and MAT.
- User role(s) information systems access as assigned.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Ensure that all clients are treated with dignity and respect.
- Affirming care for Lesbian, Gay, Bisexual, Transgender and Questioning clients.
- Perform all aspects of patient intake process including but not limited to admission- assessment, communicable disease risk assessment, CIWA Scale, COWS Scale Mental Status Exam, and toxicology testing.
-Collect toxicology samples (urine or saliva) from patients, ensuring proper labeling, handling and adherence to Universal Infection Control Precautions.
- Completion of all OASAS required admission documentation including LOCADTR and consent forms.
- Determine appropriate level of care, after reviewing case with supervisor.
- Coordinate all inpatient referrals with Intake and Referral and the Utilization Review Department.
- Communicate with other Departments (i.e. Medical, Fiscal, Intake & Referral, Clinical, Administration, etc.) to relay important clinical information.
- Provide individual, group and family counseling as needed.
- Enter Progress Notes and other Clinical documentation into EMR.
- Participate in staff meetings.
- Facilitate the completion of all necessary consent for release of information forms
- Act as liaison with community agencies to facilitate referrals
- Participate in weekly Case Conference meeting for treatment review of patients.
- Attend weekly clinical supervision.
- Ensure that Seafield’s policies and procedures and federal regulations regarding confidentiality of the patient and their records are adhered to.

EDUCATION AND QUALIFICATIONS:
CASAC or CASAC Eligible, or B/A-B/S with working knowledge of Substance Use Disorders. Working knowledge of Electronic Medical Records and Managed Care.
